Ohio RESA Guidebook 2018-2019

The Ohio RESA Guidebook includes an overview of the RESA, instructions for completing the Lesson Reflection task, a description of the Lesson Reflection domains and criteria for varying levels of performance, a glossary of terms, RESA submission and scoring policies, general systems requirements for using the submission systems, and information for modifications and accommodations.

Teaching and Learning Context Form – PDF Version

As part of the Lesson Reflection, candidates are required to complete the Teaching and Learning Context Form to provide important information about the students in the class, which helps assessors understand and interpret the Lesson Reflection.

When completing the form, we strongly recommend typing responses directly into the template. Copying and pasting formatted or bulleted text into the template can result in the modification of the template or the addition of blank pages, which will cause the form to appear blank when it is uploaded to the website.

Prior to submission, candidates are required to preview the Teaching and Learning Context Form on the Ohio RESA website to ensure the entire form is visible and legible.

Note: The form templates can be downloaded and saved as either MS Word documents (.docx) or PDFs (.pdf). If candidates try to save it as another document or convert file types (from PDF to MS Word or vice versa), the RESA Submission System will not read it.

Student Release Form

The Ohio RESA Team recommends that candidates have parents sign the following Student Release Form for incidental video capture on RESA video evidence.

RESA Modification Request Form

This form is to be completed by Resident Educators with the help of their program coordinator. Resident Educators are required to complete this form if they believe they are unable to complete the Lesson Reflection and record their lesson as outlined in the RESA Guidebook. Please note, this form does not exempt Resident Educators from the RESA, but if approved, the Resident Educator will receive a live observation modification of the Lesson Reflection. The Resident Educator will be contacted to schedule the live observation. Incomplete forms are not accepted. Submission deadline: October 10, 2018.

Please submit the completed form to OhioRESA@education.ohio.gov by attaching this form to the email.

RESA ADA Accommodations Request Form

The RESA is an untimed performance assessment that is completed online from the beginning of the 2018–2019 school year and extending to January 11, 2019. An educator with a disability who requires an accommodation in order to access the RESA Guidebook and support materials and/or requires an accommodation to complete the assessment online according to the general assessment directions may request such accommodation by completing this form.

To request approval for an accommodation and to ensure that a timely response is received, this form must be completed and submitted to TeachForward, at resa@teachforward.com by October 10, 2018.

Please note, this form does not exempt Resident Educators from completing the RESA, but if approved, modifications to the Lesson Reflection to be completed by Resident Educators will be considered.

This sample Lesson Reflection Score Report shows the new design, which features extensive and targeted feedback for each participant. You can access your score report by logging onto your TeachForward Account and clicking on "Score Report." You will be notified by email when your score report is available to view. 

Using Handbrake to Modify Video File Size and Type

HandBrake is a free software application to compress or convert video files. The vast majority of educators will not need to use HandBrake. This guide was created for educators who have tried and failed repeatedly to submit videos to the TeachForward System. HandBrake allows users to compress their video files for a faster upload process and/or to convert their video files to a format that is accepted on the TeachForward System.
